Medicaid Home Help Aide Services

Veteran Tricare Home Care Services

TRICARE is the Department of Defense (DoD) health care program for uniformed service members, retirees & their families.

​

TRICARE's home health care coverage includes part-time and intermittent skilled nursing, home health aide services, therapies, and medical social services. Pre-authorization is required, and services must be provided by a participating agency. Those with TRICARE For Life must follow Medicare rules. For overseas beneficiaries (excluding U.S. Territories), medically necessary home health care is covered, but part-time/intermittent services are not available.

To be eligible for home health aide services, individuals must be confined to their home, receive services under a physician-approved Plan of Care (POC), and be under the care of the physician who signs the POC and certification.

 State of Michigan Home Help Aide Program

Home Help Aide is a state Medicaid program administered by the Michigan Department of Health and Human Services (MDHHS) for elderly and disabled Michigan residents who require assistance with their daily and instrumental living activities that already has Medicaid or can qualify for Medicaid .

 

To enroll in Medicaid if not already enrolled please contact Michigan ENROLLS toll-free at 1-800-975-7630. TTY users may call 1-888-263-5897.

 

The office hours are Monday through Friday (except holidays) 8AM to 7PM ET. Wait times are usually shorter before 10AM, after 2PM, and later in the week.  If you have a Spenddown of Medicaid, please contact your State of Michigan case manager to see if you qualify for services or they can direct you on how to qualify.

ELIGIBILITY

To receive Home Help Services, an individual must: 

  • Have active Medicaid

  • Require physical assistance to perform one activity of daily living

Met Plus | Michigan Home Help Aide Program Steps

 Contact Met Plus by email at homehelpaide@metplus.org or 866-371-1130 and state that you’re interested in receiving home help aide services.

Make a doctor’s appointment with your primary doctor and have them complete a Medical Needs form that’s attached below. If need be, Met Plus can fax the medical Needs form to your doctor.

Have your doctor fax a copy of the medical needs form to Met Plus at 313-429-7587 and keep a copy for yourself.
